O'Flaherty, the long-serving head of Super SA, will take up his new role next month.
Announcing the appointment, Statewide chairman Nicholas Bergakis said it had followed a nationwide search to fill the chief executive role.
O'Flaherty succeeds Francis Magill in the chief executive role.
Magill's retirement after 20 years with the superannuation fund was announced in early June.
